R432-725-4 defines personal care services as: help with self-administration of medication (reminders, opening containers, RN medication set-up), transferring, personal grooming and dressing, eating and meal preparation, oral hygiene and denture care, toileting and toilet hygiene, bathing, taking/recording temperatures and weights, emergency first aid, and providing transportation. A personal care agency consists of two or more individuals providing personal care services on a visiting basis and may not exceed those services; a Transportation Services Agency does not require a Personal Care Agency license (R432-725-3). Companionship-only and general housekeeping/cleaning are not listed services, and 24-hour live-in care is not a listed category — the rule is a visiting-basis model.

The skilled door in Utah

Skilled care is the separate Home Health Agency license under R432-700 (Utah Code Title 26B, Chapter 2 licensure). Fee schedule lists Home Health Agencies at $2,138.52 plus the $743.83 two-year base (HB 8, 2024). A home health agency may also deliver personal care services; Utah Medicaid's Personal Care manual treats both licensed home health agencies and R432-725 personal care agencies as 'agencies' for PCS.

Medicaid in Utah: a second gate

Enroll with Utah Medicaid as a personal care agency (Medicaid Provider Manual — Personal Care Services, updated March 2026): PCS may be provided by a home health agency or a personal care agency licensed under R432-725, following Utah Medicaid policy, with RN assessment/supervision requirements in the manual.

The Utah Medicaid Personal Care Services manual defines EVV (electronic visit verification) as part of PCS delivery.
